El Centro’s Diabetes Self-Management Program offers a structured setting with ongoing diabetes education classes and support groups to teach the skills needed in the day-to-day management of diabetes. The program offers continuous medical care, appropriate diagnostic testing and follow up, and constant education to provide information and motivation for patients to lead healthier lifestyles. Monthly group classes for patients who have diabetes or are at risk for diabetes are taught by a certified diabetes educator (CDE). In addition, El Centro offers one-to-one visits with the CDE to improve access to diabetes self-management education. Weekly educational workshops are part of El Centro’s outreach and are offered free of charge to the community and as well as to El Centro patients, which cover topics such as nutrition, exercise, mental health (as associated with living with diabetes), medication side effects, disease complications, foot care and self-monitoring activities. El Centro’s diabetic patients also receive vouchers for free eye examinations at the University of Houston Eye Institute, and receive smoking cessation counseling through M. D. Anderson’s Ask-Advise-Connect Program.

Prevenga T2 Program

El Centro’s Prevenga T2 lifestyle change program can help you take charge of your health to prevent or delay type 2 diabetes. Prevenga T2 is part of the National Diabetes Prevention Program, led by the Center for Disease Control (CDC). It is proven to help people with prediabetes prevent or delay development of type 2 diabetes. As a part of the Prevenga T2 group, you will work with other participants and a trained lifestyle coach to learn the skills you need to make lasting changes. These changes include losing a modest amount of weight, being more physically active, and managing stress. Being part of a group provides support from other people who are facing similar challenges and trying to make the same changes you are. Together you can celebrate success and find ways to overcome obstacles.
